CANCELLATION POLICY
I. Consent for Cancellation Policy:
By scheduling an appointment, all patients hereby acknowledge and agree to adhere to the following cancellation policy.
II. 48-Hour Cancellation Requirement:
Patients are required to provide a minimum of 48 hours' notice for any appointment cancellations. Failure to do so may result in charges.
III. Cancellation Procedures:
Cancellation must be done either by calling our office or utilizing the Aesthetic Record portal for self-rescheduling. In the event of any portal-
related issues, patients are responsible for contacting our office directly for cancellation.
IV. Deposit Requirement:
A minimum deposit of $50 is mandatory to secure any appointment. The specific deposit amount may vary based on the type of service scheduled.
This deposit will be retained in case of non-compliance with the cancellation policy. Please be advised that adherence to this cancellation policy is essential to ensure the efficient operation of our scheduling system and to provide optimal service to all our patients.
